Physical Examination

Athletic Physicals are only valid for 13 months from the date of the physician's signature. Please make sure your physical does not lapse before the end of your season. Incomplete physical forms will not be valid. Please submit a complete form. Physicals turned into the nurse at registration do not go to the athletic office. Always keep an original copy of your physical.
